This Week --- IRCP Conference in Vail, Colorado

The IRCP started when about 50 delegates from Australia, Canada, Scotland and the United States came together and in Nova Scotia to share ideas on integrating rural EMS providers into rural health care delivery systems. At the meeting, they all discovered that their respective countries had similar rural health issues. It is very interesting to note that all systems which developed “Community Paramedic / Paramedic Practitioner” programs did so as the direct result of public pressure for service or a sudden change in the health care human resources dynamic of their areas. It is also interesting to note that EMS systems were not seen as part of the primary care continuum until most other options were exhausted.

Now, the IRCP is on its 6th Convention which was held at Vail, Colorado. Emergency medicine providers are the frontliners in provision of care being the first to respond to emergency calls. In cases of drug or substance toxicity, one of the first things they do inspect the area for pill bottles or chemical containers that may have been the cause of toxicity. In the absence of such evidence, they will have to rely on their assessment skills and observe the patient's symptoms to determine which substance is most likely involved.

In considering toxicological cause, there are toxic syndromes that we can watch for. Toxic syndromes or toxidromes are groups of signs and symptoms that suggest certain types of drugs or chemicals as the cause. Looking at vital signs and symptoms involving the central nervous system, gastrointestinal system, eyes, skin and mucous membranes, and genitourinary system can provide clues to the toxicological origin of the patient’s illness.

The four toxidromes that health care providers should know about include anticholinergic, cholinergic, opioid and sympathomimetic or stimulant toxidrome.

Tip This Week ---Understanding Allergies

Allergies are reactions of the immune system to certain things that does not bother most other people. Common allergens include pollen, dust mites, mold spores, certain types of food, insect stings and medications.

Allergies can cause a runny nose, sneezing, itching, rashes, swelling or asthma. Symptoms vary. Although allergies can make you feel bad, they usually won’t kill you. However, a severe reaction called anaphylaxis is life-threatening.

There are tests that can be done to determine if a person is hypersensitive to a a substance. Types of tests are in vivo or skin/scratch test, in vitro or the measurement of specific IgE antibodies, and provocation testing where an allergen is introduced one at a time and the subject is observed for presence of reactions.

Though allergic reactions can resolve on its own, the symptoms that it produces may be rather discomforting. Medications that can be given to resolve hypersensitivity include Corticosteroids, Antihistamines, Decongestants, and other medications that work by blocking symptom-causing chemicals released during an allergic reaction such as Montelukast, Cromolyn Sodium and Mast cell stabilizers.

Immunotherapy or allergy shots can also be done to desensitize the patient to decrease or eliminate the need for medications. For patients with severe allergic reactions or anaphylaxis, epinephrine shots may be given.

This Month's MedicCast Live Show EMS Radio Consults


A special thanks to Dr. Ginger Campbell of the Brain Science Podcast among other shows (find Ginger's stuff at GingerCampbellMD.com). She suggested the topic for this week's episode and was gracious enough to come on the show live and share her thoughts about receiving radio patient information in the ER.

Also, a thank you to Dr. Craig Corey for taking the time to write his thoughts down even though he couldn't be on the show live. Craig is the inventor of the OxyPhone pediatric nebulizer device. You'll find that and Dr. Corey's home page at OxyPhone.com.

The central point brought out by both of these ER docs was that we must get to the point of the call quickly, focusing on the key problem, immediate life threats and interventions taken, time to the hospital, etc. and then let the ER ask any follow up questions. Vitals are important but not as important as patient's chief complaint and your impression of the stability of the patient.

Ginger also stressed the importance of talking to the physician after the call to learn from what you saw or did during the transport to the hospital. She said that her respect for the EMT or paramedic rises significantly when they want to learn from the calls - good and bad.

Children are naturally curious with the objects they see around them. Most often, whatever they pick up goes straight to their mouth which makes them prone to things like choking, GI disorders and yes, poisoning.

To be able to initiate interventions, it is important to know the agent that the child ingested. EMS providers should look around and be aware of the objects in the child's environment and the substances found that they could possibly ingest. It is also important to assess carefully for the different signs and symptoms that the child is exhibiting. If the child is symptomatic, obtaining the history of ingestion is also helpful.

Since lead and iron are some of the most common substances that cause pediatric poisonings, providers should also be aware of the signs and symptoms and emergency treatment of lead and iron poisoning. Teaching the parents or caregivers about how and where they can get poisoned by these substances, how they should store chemicals, as well as how they educate the children about medications would be key to the prevention of pediatric poisonings.

]]> Sun, 30 May 2010 21:02:25 -0400 ems-review-on-atrial-flutter-and-episode-221 Jamie Davis, the Podmedic EMS Review on Atrial Flutter and Episode 221 of the MedicCast EMS podcast for EMT, paramedic and other emergency medical services providers, nurses, and doctors Tip This Week --- Atrial Flutter Atrial flutter is a rhythm disorder involving a rapid heart rate, in which the upper heart chambers (atria) are stimulated to contract in a very disorganized and abnormal manner. Though atrial flutter is somewhat similar with atrial fibrillation, the underlying mechanism of atrial flutter makes it amenable to cure this arrhythmia with percutaneous catheter-based techniques In atrial fibrillation, the atria are stimulated to contract very quickly and differently from the normal pattern. The impulses are sent to the ventricles in an irregular pattern. This makes the ventricles beat abnormally, leading to an irregular (and usually fast) pulse. In atrial flutter, the ventricles may beat very fast, but in a regular pattern. Causes of atrial flutter include cardiac issues like RHD, CHD, left ventricular dysfunction and postcardiac surgery. Other disorders like thyroid disease, obesity, pulmonary disease and PE have been associated with atrial fibrillation and flutter. Emergency treatment of atrial flutter are aimed at getting the heart back to normal rhythm through administration of intravenous (IV) drugs such as dofetilide, amiodarone, or ibutilide. Long-term treatment varies depending on the cause of the atrial fibrillation or flutter. Medications to slow the heartbeat may include beta blockers, calcium channel blockers and digitalis. Blood thinners are also given to reduce the risk of blood clots. They are very commonly prescribed and very commonly involved in unintentional as well as intentional overdoses. EMS providers looking at patients with symptoms of overdose include confusion, dizziness, confussion, arrhythmia, slurred speech, bradycardia, SOB and weakness should ask if the patient is taking calcium channel blockers and suspect overdose if they are. To resolve the symptoms, protocols for the treatment of bradycardia and hypotension should be followed, and may include atropine, epinephrine, norepinephrine, other vasopressors and IV fluids. Calcium and Glucagon may also be given as antidotes to calcium channel blockers. For patients following treatment of overdose, calcium levels should be monitored and observed for complications of overaggressive calcium administration. Dave Pauli is the ISR Matrix International Australian representative, commenced his career in the Australian Defence Force in reconnaissance, force protection, leadership and instructional roles. In this case, rapid, disorganized electrical signals cause the atria, the two upper chambers of the heart, to contract very rapidly. To better understand how atrial fibrillation happens, paramedics and EMS students should brush up on their knowledge in cardiac anatomy and how electrical conduction in the heart works. The incidence of this condition becomes more common with age. Atrial fibrillation can be caused by alcohol use, CHF, CAD, heart surgery, hypertension, pericarditis, medications and valvular heart disease. Emergency medicine providers who assessed patients with heart risks suspect atrial fibrillation if the patient presents with palpitations, SOB while lying down, confusion, dizziness, light headedness, fainting and fatigue. The health care provider may hear a fast heartbeat while listening to the heart with a stethoscope. The pulse may feel rapid, irregular, or both. The normal heart rate is 60 - 100, but in atrial fibrillation/flutter the heart rate may be 100 - 175. Blood pressure may be normal or low. An ECG shows atrial fibrillation or atrial flutter. Tip This Week --- Heat Related Emergencies Summer is fast approaching so emergency medicine providers should be ready to respond to emergencies related to temperature spikes like heat stroke, heat cramps and heat exhaustion. Heat stroke occurs when the body becomes unable to control its temperature. The body's temperature rises rapidly, the sweating mechanism fails, and the body is unable to cool down. Body temperature may rise to 106°F or higher within 10-15 minutes. Heat stroke can cause death or permanent disability if emergency treatment is not given. Apart from rapid increase in body temperature, other symptoms include rapid, strong pulse, throbbing headache, dizziness, nausea, and confusion. To increase the chances of a person having a heat stroke, the patient- if able, or another person should immediate call for help and while waiting for emergency responders should make attempts to cool the person's temperature by taking them out of direct sunlight and take them to a cool place, immersing or spraying the patient with cool water. Tip This Week --- EpiPens and Epinephrine Reviewed Anaphylaxis is a sudden, severe allergic reaction that is life-threatening. Tissues get inflamed, including the airway, which impedes the entrance of oxygen. Other symptoms include hives, skin redness, high pitched breathing sounds, palpitations, nasal congestion and difficulty breathing. Since anaphylactic reactions occur suddenly, a fast acting medication to reverse the life-threatening effects should be given. Epinephrine is an emergency drug that acts quickly to improve breathing, stimulate the heart and reverse hives and swelling around the face and lips. This medication is used in emergencies to treat potentially dangerous allergic reactions to insect stings or bites, foods, drugs or other substances. EMS providers should know the proper administration, interactions, precautions and most importantly, their local EMS protocols for the use of this medication. The FDA announced yesterday that the Lifepak 15 cardiac monitor is under a class I recall based on reports that the unit will power on or off spontaneously. Specifically the following reported problems have been documented:

• Power Off then On by itself.
• Power Off then NOT turn On.
• Power Off by itself requiring the operator to turn it back On.
• Stay powered On and not allow itself to be turned Off.

Physio-Control, Inc. who manufactures the cardiac monitor/defibrillator, reportedly sent notices to customers with these monitor defibrillators on March 4, 2010. In the letter were instructions advising them to keep the device in service and to test their devices according to the operating instructions.

The company stated that local service representatives are scheduling service visits within 60 days. Customers were asked to forward the Physio-Control notice to all their responsible parties.

The FDA press release had this statement about the class of recall involved.

Class 1 recalls are the most serious type of recall and involve situations in which there is a reasonable probability that use of these products will cause serious adverse health consequences or death.

Health care professionals and consumers may report adverse reactions or quality problems they experienced using these products to the FDA’s MedWatch Adverse Event Reporting program either online, by regular mail or by FAX.


You may remember that Physio-Control recently came out from under an FDA consent decree restricting their sales of certain devices due to quality control paperwork deficiencies. You can listen to my interview with Cam Pollock, VP of Marketing at Physio-Control recorded at EMS Today live in March.

This is just another in the recent spate of recalls of equipment in the EMS realm including recalls of Cardiac Science AEDs. The increased scrutiny on emergency medical services equipment now makes me wonder if there has been a laxity on oversight in our market with our equipment and gear in that past.

K2, Fake weed, spice, and zohai are just some of the names given to synthetic marijuana. It is a chemical that is intended to substitute marijuana and mimics its effects only more potent. It may be a mixture of herbal and spice plant products, but it is sprayed with a potent psychotropic drug and likely contaminated with an unknown toxic substance that is causing many adverse effects. Unlike marijuana, reactions from using K2 include agitation, anxiety, tachycardia, and elevated blood pressure. There are also synthetic/designer drugs out there like K2 that are contaminated with unknown substances and untested substances making it difficult to identify and predict its effects. Tip This Week --- EMS Review on Methicillin-Resistant Staphylococcus Aureus MRSA, otherwise known as methycillin-resistant staphylococcus aureus, is a type of staphylococcus bacteria that is resistant to certain types of antibiotics. It can either be hospital associated (HA-MRSA) which happens in the healthcare setting or community acquired (CA- MRSA) which is passed through skin contact with infected people. MRSA may present as a skin infection or a bump which may be red, swollen, painful, warm to touch, with pus or drainage and accompanied by fever. To help prevent the spreading MRSA to other patients or acquiring the infection themselves, providers of emergency medicine should always do precautionary measures when handling patients like avoiding contact with wounds and drainage, disinfection of soiled or contaminated articles or equipment and most importantly, remember to practice good hand hygiene before and after handling patients. Tim Phalen, one of the premier 12 lead educators in the field, Tom Bouthillet from the Prehospital 12 Lead ECG blog, and Angel Burba from Howard Community College all joined me on the show to discuss 12 lead ecg's. We talked about some of the common missteps and educational hurdles that providers see when learning or expanding on their knowledge of them. Tim brought up the very important point that we should never just take A 12 LEAD. The importance of trending the patient means that we should be taking multiple 12 leads to watch for changes in perfusion, ischemia, and progression of MI in a STEMI patient. Tom Bouthillet pointed out that ignoring axis determination removed a very important tool from our 12 lead interpretation skill set. He sees this all too often in the providers he talks with both in person and via his blog. Angel Burba answered questions about how 12 lead is taught to new paramedics and some of the challenges she and other educators see with teaching 12 leads to new providers. She stressed that one issue is to show where the 12 lead ecg falls along the patient assessment and care process. Cam graciously agree to sit down and talk with Chris Montera of the EMS Garage Podcast and myself about the recent lifting of the consent decree by the U.S. Food and Drug Administration. The consent decree had restricted sales of Physio-Control products while a review of QA in their manufacturing process was conducted.

This was a rare opportunity to talk with an industry "insider" about some of their challenges with customer support during the restricted time period and also to find out more about Physio's plans now that the decree has been lifted.

On the show, I had James Weber, an associate member of the Sudden Cardiac Arrest Association and a long time friend of the MedicCast. James is active in his community as a paramedic in Lancaster County, Pennsylvania and works with others to provide more access to AEDs in this rural community in the southeastern part of the state. One of the initiatives he discussed was a program that distributed AEDs within the Amish community. Also, joining us, at long last, is Amy Turley. Amy is a paramedic in the Seattle, Washington region but is probably most well known for developing the Turley Backboard Pad, an innovative solution for padding the pressure points of a boarded EMS patient as well as a providing a tool for both warming or cooling a patient. Amy talks about the differences between the TRAUMA TRIAD of DEATH and the therapeutic use of hypothermia in cardiac arrest and stroke patients. Finally, Mary Newman is representing the Sudden Cardiac Arrest Foundation, another organization focusing on survivor stories and providing resources for research and media about sudden cardiac arrest. Tip This Week --- Interview with Richard Brooks and Rick Russotti on Snow Response in Cecil County Maryland

I had the distinct pleasure to sit in on an interview between Rick Russotti of the Mitigation Journal Podcast and Richard Brooks, Director of Emergency Services for my own response area, Cecil County, Maryland. They were discussing snow response preparedness in the wake of the recent rash of snow events this year in the Mid-Atlantic region.

For the EMS provider on the street, whether you are an EMT, medic, paramedic or fire-fighter, it is often our lot in life to sit and grumble about how resources are allocated during major (and minor) events. I think we don't know or care to know the why's and wherefores of such decisions by our emergency managers and EMS supervisors.

Perhaps this segment will help you better understand the ways that preparedness planning occurs, how your supervisors and planners make their choices on types and levels of emergency medical services response, and what you can do to make their jobs (along with your own) a little easier.

Falls in children and adolescents tend to occur in homes, followed by schoolyards and playgrounds. Adults in the other hand tend to get injured from occupational falls. A large percentage of falls in adults have been associated with alcohol use. Older adults aged 65 and above are highly at risk for falls because of factors like poor muscle tone, poor balance, medication use, and frequency of falls increase from environmental factors including poor lighting and lack of safety devices. In assessing fall injuries, EMS providers could determine the extent of injury resulting from falls depending on certain factors like the distance of the fall, the landing surface, orientation on falling, and whether the fall was broken. Emergency responders often see patients who have used street drugs like heroin and cocaine but don't always think about what other drugs or substances that have been mixed in with them. These contaminants are usually added to dilute the drug or to extend the "high". These may or may not change the appearance and taste of the drug and may prove difficult for the user or paramedic to immediately identify the contaminants. For providers attending to patients on street drugs who are presenting unusual signs and symptoms, it would be advisable to coordinate with their local poison centers so they could project what other drugs may have been added. For EMS providers to determine when not to resuscitate, one thing they could do is to check with their state laws. Some states have specific provisions for DNR orders like in form of bracelets, necklaces or cards. Resuscitation can also be withheld in cases like decapitation, decomposition, and trauma where survival is not possible. DNR can also be considered in patients suffering from a terminal illness like cancer in advanced stage. In cases patients do not meet the criteria for DNR (as per state law), EMS providers should initiate all needed interventions for patients in need of resuscitation. ]]> Sun, 13 Dec 2009 21:57:10 -0500 buprenorphine-overdose-with-lisa-booze-and-episode Jamie Davis, the Podmedic Buprenorphine Overdose with Lisa Booze and Episode 197 of the MedicCast EMS program for EMT and paramedic EMS providers Tip This Week --- Buprenorphine Overdose with Lisa Booze Buprenorphine, more popularly known as Subutex, is an opioid antagonist used to treat opioid dependence such as addiction to narcotic analgesics, heroine, or any drug with an opioid formulation. Suboxone, is buprenophrine added with naloxone to decrease the likelihood of diversion and abuse of the combination product. Ideal candidates for Buprenorphine therapy are chosen following a criteria and three phases of maintenance therapy are followed. The dose of Buprenorphine given in each phase of the therapy is closely monitored by the physician and adjusted through the progression of each phase until medically supervise withdrawal (formerly called detoxification) is achieved. providers responding to Buprenorphine users with pinpoint pupils, extreme drowsiness, dizziness, blurred vision and slowed breathing should suspect drug overdose. Emergency measures should be directed at providing a patent airway for the patient, careful monitoring of respiratory and cardiac status and immediate transport to a health facility. If overdosed with parenteral form of the drug, paramedics should remember that Naloxone may not be effective in reversing respiratory depression. Doxapram, a respiratory stimulant, may be used. ]]> Sun, 06 Dec 2009 16:44:53 -0500 stemi-imitators-for-ems-and-episode-196 Jamie Davis, the Podmedic STEMI (S-T Elevation Myocardial Infarction) Imitators Review for EMS and Episode 196 of the MedicCast EMS Podcast for EMTs Paramedics and other emergency medical services providers Tip This Week --- False Positives for STEMIs Paramedics and medics (EMT-I) treating cardiac patients in the field are often taught to look for and diagnose S-T elevation myocardial infarctions (STEMI) in the field. EMS providers do this using their heart monitors and performing a 12-lead electro cardiogram (ECG) as well as adding in information from physical assessment and patient history. With more awareness about STEMI recognition and treatment, some cardiac centers are noticing a larger number of "false positive" STEMI patients coming in to their cath labs. The question is now being asked, are we ruling out STEMI imitators carefully enough in the field and in community hospitals. I add the local hospitals because the local ER docs may make the same mistaken diagnosis based on presentation and ECG evidence, too (based on evidence from one article linked below). The two easiest STEMI imitators are the bundle branch blocks. Left bundle branch blocks almost always present with an S-T segment elevation, and while Right bundle branch blocks usually present with S-T segment depression, this may not always be the case. Bundle branch blocks are most easily identified by the presence of a wide QRS segment (greater than .17 seconds). While any patient presenting with symptoms of MI including chest pain, shortness of breathe, diaphoresis, and other signs of shock who has a wide complex QRS segment on their ECG with ST Elevation MIGHT indeed be having an MI - the ECG S-T segment is not diagnostic for this and your system may have to rely on cardiac markers in the blood tests to make a decision. Other typical STEMI imitators include Left Ventricular Hypertrophy, Pericarditis, and Ventricular Aneurysm. Tip This Week Pulmonary Embolism

Pulmonary Embolism is a sudden blockage in a lung artery. It is usually a result of a blood clot in the leg (DVT) that broke loose and traveled into the lungs via circulation.

About half the people who have pulmonary embolism do not show any symptoms, but clinical manifestations include shortness of breath, chest pain or coughing up blood. Treatment is aimed at breaking up the blood clots and preventing formation of new clots.

The most important thing that can be done in the prehospital setting is to transport the patient to a hospital. As long as no reliable method is available of making a clinical diagnosis of pulmonary embolism (PE) without diagnostic tests, treating PE in a meaningful way in the field will remain difficult. Emergency interventions are aimed at respiratory management and establishment of an IV line.

]]> Sun, 22 Nov 2009 21:34:39 -0500 deep-vein-thrombosis-dvt-and-episode-194 Jamie Davis, the Podmedic Deep Vein Thrombosis (DVT) and Episode 194 of the MedicCast EMS podcast for EMT and paramedic emergency medical providers and students Tip This Week- Deep Vein Thrombosis (DVT) Deep venous thrombosis (DVT) refers to a blood clot embedded in one of the major deep veins of the lower legs, thighs, or pelvis. A clot blocks blood circulation through these veins, which carry blood from the lower body back to the heart. The blockage can cause pain, swelling, or warmth in the affected leg. If this clot is left unresolved, it may dislodge and travel to the lungs via the bloodstream, a more serious problem called pulmonary embolism. Physical symptoms of DVT are similar with the symptoms after the occurrence of trauma. To be able to identify DVT, paramedics should use their critical thinking skills, thorough history taking as well as knowledge on the causes and risk factors. If there is a moderate or high clinical suspicion of DVT an injection of low-molecular-weight (LMW) heparin in doses designed to treat acute DVT can be given. Diagnostic imaging can then be arranged on a more elective basis the following day. In using a stethoscope, it is important that proper placement and technique is done to be able to maximize the use of the device. Selecting an ideal stethoscope that will fit the preferences of t user will also help in the quality of its function. The areas most often auscultated are the lungs and the heart. To be able to identify abnormalities, the provider should be trained and be familiar with the different breath sounds and heart sounds. The field or environment where the assessment is done won't always be ideal and much noise can be present interfering with the sound they are trying to hear. Discussion on New Paramedic Show "Trauma" Last Monday night, members of the EMS community got together online to watch the premier of the new show Trauma. Comments were mostly made on how TV paramedics portray the real paramedics and the accuracy of the procedures that they do. For EMS providers, it is important that these kind of shows get as close to the real thing as possible to educate the public about what emergency medicine is all about and not give them false impressions or expectations. Together with the podmedic were EMS podcasters Greg Friese of EMS Educast and Chris Montera of the EMS Garage with a paramedic consultant from the show as a special guest. One of the actresses, Aimee Garcia (facebook fan page), called in to share her experiences during her preparation for her character portrayal. She said that actual paramedics provided the EMS training for the actors which included the basic EMS procedures. She speaks highly of the profession and that experience made her understand more about what paramedics do and that she will suggest to the production team all the comments from the show to make it as real as possible and they will work hard to do EMS providers justice. no 1:04:46
