This Week: EMS Culture of Safety Wrap Groups: Operations and Crew Resource Management, Vehicle Safety

Here comes the final part of the warp up from the EMS Culture of Safety conference held in Arlington, Virginia on June 21-22, 2011. In this episode groups four and five.

The fourth group started off discussing their assigned topic operations and crew resource management, touching on specific subjects such as scene operations, bystander’s safety response policies, situational awareness and team work and scheduling and communications. On that note, they came up with questions like where is EMS now, where does EMS want to be, what do they want to do in a strategy perspective, and what are the obstacles and barriers and answered those questions to put things into perspective.

The fifth group followed with their topic on vehicle safety tackling aspects like vehicle design and provider practices when on board the ambulance. One of the things that came up was the provider culture or using seat belts and finding ways to secure equipment and passengers alike while providing care. They recommended was putting out a mandate that requires providers to use seat belts and a possible obstacle that can hinder this progression EMS would be attitude and culture.

This Week: EMS Culture of Safety Wrap Groups: Provider Wellness, Patient Safety, Provider Safety

Bringing the first three segments of the final day wrap up from the EMS Culture of Safety conference held in Arlington, Virginia on June 21-22, 2011. After the first day's presentations on safety cultures in other industries and data from various EMS studies, the attendees broke out into groups to work on distinct topics surrounding safety in and around EMS.

The first breakout group to present their brainstormed ideas and key points focused on EMT and paramedic provider health and wellness. A healthy workforce is a safer workforce. Working from that premise, the focus was on ways to improve the overall health of the EMS workforce is an important part of any culture of safety change. The team focused on ways to use positive reinforcement and rewards to encourage changes in lifestyles and behavior.

Second up was the group focused on patient safety, especially medical errors by providers. Using the concepts of "Just Culture" that creates an environment of openness and educational opportunities around errors and reporting, the team hopes to improve the ways providers look at errors and opportunities and move away from disciplinary, personal attack based systems of error reporting and response.

The third segment to wrap up this week's show looked at EMS Provider Safety. While this is often what we think of first in EMS Culture of safety, it was only one of 5 areas discussed at the conference. for the final three EMS Culture of Safety segments from the final day wrap up session, check back here at the MedicCast next week for the final session from this conference.

This Week: Panel Discussion on Safety Research at the National EMS Culture of Safety Conference

This episode is the continuation of the podmedic and co-podcaster Carissa O’Brien of Baseline Vitals, interview segments as they traveled to Arlington, Virginia to attend the National EMS Culture of Safety Conference. Here they have a discussion with American Airlines' 12-year chief safety officer, Scott Griffith. Although not involved in any particular field of EMS or medical service, Scott was part of the community of aviation leaders that developed the "Just Culture" concept which refers to notions of justice that is how to be fair and consistent in how we respond to human beings who work in high risk industry such as EMS.

Later on in the discussion, they were joined by prehospital researcher Blair Baygum and EMS 10 award winner Daniel Patterson of University of Pittsburgh. They highlight what they thought was important for EMS safety and patient safety. They packaged that presentation in a set of myths and provide some data to dispel those myths and also provided a frame of reference for some of the data that exist out there that characterizes one or more attributes of safety culture in EMS versus other settings. They gave the audience basically a frame of reference of where EMS might be and where EMS might want to go.

Tip of the Week: Accidental EpiPen Injections Tox Segment with Lisa Booze

The EpiPen is an emergency shot of epinephrine given to individuals experiencing anaphylaxis or an intense allergic reaction due to insect stings or bites, foods, drugs and other allergens as well as idiopathic or exercise-induced anaphylaxis. This can be kept by individuals with a history of anaphylaxis in the past and can be self-injected when the need arises.

What happens if an EpiPen is accidentally injected to an individual who is not really in need of it? Tip of the Week: NREMT Cardiac Skills Part 2

The heart has an internal electrical system that controls the rhythm of the heartbeat. Problems can cause abnormal heart rhythms, called arrhythmias. There are many types of arrhythmia. During an arrhythmia, the heart can beat too fast, too slow, or it can stop beating. Sudden cardiac arrest occurs when the heart develops an arrhythmia that causes it to stop beating. This is different than a heart attack, where the heart usually continues to beat but blood flow to the heart is blocked.

In this week's follow up episode from the previous NREMT Cardiac Skills Part 1, the podmedic provides additional tips for the NREMT skills for cardiac arrest management as well as tips and what to remember when taking this skills station.

Tip of the Week: NREMT Cardiac Skills Part 1

The heart has an internal electrical system that controls the rhythm of the heartbeat. Problems can cause abnormal heart rhythms, called arrhythmias. There are many types of arrhythmia. During an arrhythmia, the heart can beat too fast, too slow, or it can stop beating. Sudden cardiac arrest occurs when the heart develops an arrhythmia that causes it to stop beating. This is different than a heart attack, where the heart usually continues to beat but blood flow to the heart is blocked.

A person who suffered from cardiac arrest will die within a few minutes without medical intervention. Patient outcomes depend on the time medical intervention has been initiated and on the skills of the first responder who provided initial cardiac arrest management. Moving on from the previous week's discussion on airway management skills, this week's tip of the week will be on cardiac skills or emergency management of cardiac issues.

In this episode, host Jamie Davis, the Podmedic brings Community Emergency Response Team (CERT) educator, Steve Lichtenberg from Montgomery County, Maryland on the show to talk about his efforts to train CERT volunteers in Haiti and their effectiveness following the devastating earthquakes last year.

All emergency responders in a major disaster will be sorely tested in time and resources and these community-based volunteers play a significant role in both preparing for disaster prior to occurrence and in the immediate response after a disaster strikes in the crucial first hours before state and national resources can be mobilized.

Tip of the Week: Caffeine Overdoses Toxicology Segment

Caffeine is a stimulant, producing alertness, restlessness and hyperactivity, but it’s also a cardiovascular stimulant, causing tachycardia and an increase in blood pressure.

Other than coffee, drinks containing caffeine include tea, cola soft drinks and energy drinks. The most common symptoms following the ingestion of large doses are nausea and vomiting, which can be severe and protracted. Caffeine is a diuretic so consuming large amounts can lead to dehydration. With very large doses, more serious effects can occur, including dysrhythmias such as supraventricular tachycardia, atrial fibrillation, and ventricular tachycardia, seizures, metabolic acidosis, low potassium levels and cardiovascular collapse.

Tip of the Week: EMS Today Finale Part 1

This is the first of a two-part finale episode from the EMS Today conference in Baltimore. The Podmedic Jamie Davis and Chris Montera of EMS Garage come together and interview Andy Lovell, GCEMS Chief and discuss Aladtec's EMS scheduling product called EMS Manager which is basically a web based service also available on a smart phone that aids in scheduling of hundreds of providers in an EMS system.

Another guest that come on the show was Dr. Anonymous and Dr. Meyers and get on a discussion on EMS and emergency medicine. Keith introduced a product called BEAR (Bariatric Equalizing Abdominal Restraint) which functions by turning a regular cot into a bariatric cot by keeping the abdominal mass centered into the cot, diminishing the risk for tipping because of weight shifting especially while in transport. Josh from Wantynu also talks about the features of the TK3, an oxygen tank wrench designed to operate in a wider environment.

Tip This Week- Bronchodilators

Over the coming weeks, we will be providing reviews on the most commonly used prescription medications in the United States. This week, we will cover on the top prescription drug used by individuals aged 0 to 11 years old, Bronchodilators.

Bronchodilators dilate the bronchi and bronchioles, decreasing resistance in the respiratory airway and increasing airflow to the lungs. They are most useful in obstructive lung diseases, of which asthma and chronic obstructive pulmonary disease are the most common conditions.

Some bronchodilators are inhaled, using a nebulizer or an inhalation aerosol.Others are taken as injections or by mouth. Examples of prescription bronchodilators are albuterol (Proventil, Ventolin), epinephrine (Primatene), ipratropium (Atrovent), metaproterenol (Alupent, Metaprel), and terbutaline (Brethine). A few bronchodilators, such as ephedrine, can be bought without a physician's prescription. However, even over-the-counter bronchodilators should only be used as recommended by a physician.

Tip This Week- EMS Review on Initial Assessment, Part 1

The very first thing an emergency medical service provider should do when responding to a call is assessment. Patient outcomes are highly affected with how well the provider does his assessment and his decision making. For new EMS providers on the field, it takes a keen observation and focus to be able to perform a clear assessment amidst all the stress, tension and anxiety happening around him.

Here are some of the important things to look at in performing an initial assessment:

A- Airway

* no airway
* snoring
* gurgling
* vomiting
* blood in the airway

B- Breathing

* no breathing
* wheezing
* labored breathing
* use of accessory muscles
* retractions
* open hole in chest

C- Circulation

* pulses
* perfusion
* capillary refill

D- Disability or Mental Function

* level of consciousness
* GCS
* consider presence of cervical spine injury
* A- alert, awake, aware of person, place and time
* V- responsive to verbal stimuli
* P- responsive to painful stimuli
* U- unconscious, unresponsive to stimuli

Tip This Week- Recognizing Stroke and Early Treatment Considerations

Stroke is occurs when part of the blood supply to the brain in interrupted as in ischemic stroke or blood vessels burst and blood pools into the spaces of the brain or hemorrhagic stroke. Signs and symptoms of stroke include sudden numbness or weakness, especially on one side of the body; sudden confusion or trouble speaking or understanding speech; sudden trouble seeing in one or both eyes; sudden trouble with walking, dizziness, or loss of balance or coordination; or sudden severe headache with no known cause.

Early management and diagnosis of a stroke is essential to a good prognosis. One must consider the presence of risk factors that could predispose an individual to a stroke event such as chronic hypertension, atrial fibrillation and diabetes. Acute stroke therapies try to stop a stroke while it is happening by quickly dissolving the blood clot causing an ischemic stroke or by stopping the bleeding of a hemorrhagic stroke. Medication or drug therapy used to prevent or treat stroke are antithrombotics (antiplatelet agents and anticoagulants) and thrombolytics.

Health care providers are at a very high risk for acquiring infection because of the work environment that they have. More importantly, they are also the best vectors for the spread of hospital acquired infections. As defined by the CDC, Healthcare-associated infections (HAIs) are infections caused by a wide variety of common and unusual bacteria, fungi, and viruses during the course of receiving medical care.

Examples include sustained reduction in central line-associated bloodstream infections (CLABSIs) by 70%, simply by ensuring adherence to available guidelines. Reductions have been demonstrated for other HAIs as well, but, much more remains to be done. Wherever patient care is provided, adherence to infection prevention guidelines is needed to ensure that all care is safe care. This includes traditional hospital settings as well as outpatient surgery centers, long-term care facilities, rehabilitation centers, and community clinics.

People often call poison centers or 911 after ingesting or being exposed to chemicals or organic substances that are believed to be toxic or poisonous. However,these substances that are mistaken to be toxic, only cause GI upsets and are not lethal at all. Common household items like petroleum jelly, silica gel, Poinsettia, toothpaste and bleach are just some of them.

As providers of emergency medicine, we should know about what’s toxic and what’s not to be able to educate the public on which items they should be extra careful with, how to handle GI upsets, and also reduce anxiety and panic especially in parents with children who accidentally ingested chemicals that are actually non-toxic and also inform the public of the availability of poison center hotlines to get in touch with individuals who are well versed with poisoning and can provide them with better resources and information.

Tip This Week- Extrapyramidal Symptoms

A major problem for patients taking antipsychotic medications is the occurrence of side effects which include akathesia, dyskinesia, dystonia and pseudoparkinsonism. The clinical manifestations include a number of atypical involuntary muscle contractions that influence gait, movement, and posture. The symptoms can develop acutely, be delayed, or overlap making diagnosing a challenge.

Extrapyramidal side effects and symptoms vary and may include:

  • Movement changes, such as:

    • Involuntary movements
    • Slowed movements
  • Increased muscle tone
  • Muscle spasms and muscle rigidity
  • Memory loss
  • Problems finding words
  • Tremor
  • Uncontrollable, repeated movements, speech, or cries (tics)
  • Walking difficulty

Appearance of these symptoms plus a medication history of antipsychotic durgs is highly indicative of Extra pyramidal symptoms. To know more about EPS, follow the links provided below.

Recorded LIVE from EMS Expo 2010 in Dallas, TX

The final of three MedicCast Episodes recorded at EMS Expo 2010 in Dallas Texas, this show featured a discussion with three of the top proponents in the U.S. for Community Paramedicine programs. Host Jamie Davis, the Podmedic, was able to bring to in Chris Montera, Chief of the Western Eagle County Ambulance District (also host of the EMS Garage podcast), Eagle County Colorado public health nurse manager Anne Robinson, and Gary Wingrove, one of the founders and organizers of the International Roundtable on Community Paramedicine (IRCP.info).

Anne and Chris had recently started their community paramedicine program in Western Eagle County including initiating a specific paramedic training program and applying the public health needs assessment to their community in light of a paramedic’s existing skill set and scope of practice. They were interviewed this summer here on the MedicCast about getting started with their community paramedicine program.

This episode follows up on their first group of patients, successes, failures and the next steps moving forward. It is very exciting to see what they are doing with this program and specifically how they have integrated it into the existing structure of primary and clinic care in their community. ]]> Sun, 05 Dec 2010 21:38:43 -0500 pediatric-cough-and-cold-meds-revisited-with-lisa Jamie Davis, the Podmedic From 1990 to 2006, as many as 10% of children in the United States were given cough and cold medicines weekly for illnesses and cold-like symptoms. A few years ago, a group of physicians petitioned the Food and Drug Administration to relook at the safety of pediatric cough and cold medicines. What prompted this was the fact that there were more than 60,000 possible poisonings reported to poison centers yearly, coupled with the lack of any data that these products were effective in small children. In 2007, soon after the petition, manufacturers withdrew over-the-counter cough and cold products that were marketed for children less than 2 years old. In 2008, the FDA recommended that cough and cold products not be used at all in children under 2. Also in 2008, manufacturers went further in response to public pressure, and changed the labeling of products to indicate that they should not be used in children less than 4 years old. The FDA supported this action. A study that was published in Annals of Emergency Medicine in 2009 looked at pediatric fatalities associated with over-the-counter cough and cold meds reported by 5 sources, including poison centers, over a period of at least 20 years (different sources included different time periods), up to the time of the actions taken on these products. There were just 118 deaths in children less than 12 years old that were concluded to be possibly due to cough and cold medicines. Most of the fatalities were in children less than 2 years of age. Eighty-eight of the deaths involved an overdose of the medicine, and most of the remaining cases involved giving the child more than one product with the same ingredients. Unfortunately, many of the overdoses were when adults gave the children the cough & cold medicines to sedate them or to control their behavior. This sheds a little more light on pediatric fatalities with cough and cold medicines – how many and why they occurred, prior to the regulatory and voluntary actions. The FDA is reviewing whether to recommend more restrictions on the labeling of over-the-counter cough and cold medicines. Some have raised concerns that placing restrictions on cough and cold medicines for children might actually increase overdoses, hospitalizations and severe outcomes by parents giving their young children products and doses that are intended to be used by older children and adults. So, Dr. Wendy Klein-Schwartz and Dr Suzanne Doyon of the Maryland Poison center along with Dr. David Sorkin of the University of Maryland School of Medicine analyzed poison center data since the recall and limitations took effect to see if this concern is a valid concern. Their research was published in the August 2010 issue of Pharmacoepidemiology and Drug Safety. The authors found that there was a greater than 50% decrease in the rate of cases of therapeutic dosing errors in children less than 2 years old for the 15 months following the withdrawal in October 2007. This time period included a flu season that was more severe than previously, leading one to predict that there would be more cases of parents giving children cough and cold medicines. So, concerns that restrictions would lead to more harm is not supported. Lastly, remember that your regional poison center is available 24/7 to assess and recommend treatment for all cough and cold medicine exposures in children. This Week-- Antipsychotic Medication Review for EMS

One of the more commonly prescribed type of these medications are anti-psychotics. These medications help patients with psychosis lead a more normal and fulfilling life by alleviating such symptoms as hallucinations, both visual and auditory, and paranoid thoughts. However, there are side-effects accompanying these medications.

Side effects that may be seen in patient taking antipsychotic medications include drowsiness, rapid heartbeat, blurred vision, photosensitivity, skin rashes and dizziness when changing position. Anti-psychotics are further classified into subtypes, the more recently developed antipsychotic medications have much lesser side effects though the earlier released type is still being used. Interview with Tom Bouthillet

In this episode, the podmedic Jamie Davis does an interview with Tom Bouthillet of the Prehospital 12-Lead ECG blog and they talk about the different changes in prehospital management particularly CPR, cardiac care, and some EMS protocols.

Specifically, Tom and Jamie discuss ways EMTs and paramedics can begin to implement the spirit of the ECC 2010 guideline changes and what might better be called "reinforcement." The two also talk about the challenges associated with tracking and planning for success as an EMS professional when dealling with sudden cardiac arrest in each community.

Whether your emergency medical services system is large or small, it is imperative to track your numbers for sudden cardiac arrest patients. Only by tracking and following your efforts can you begin to find areas in which you can improve your patient outcomes. Tom Bouthillet particularly likes the Choreographed approach to cardiac arrest management which he thinks is more like a NASCAR Pit Crew than a dance troupe.

When every team member knows what they are doing and how they are to interact with the rest of the team, each EMT and paramedic can effectively work towards giving that patient the best chance for survival.

Discussion of 12 Lead and Acute Coronary Syndromes (ACS)

In this very special episode of the MedicCast, Jamie Davis, the Podmedic hosts a panel discussion with some online EMS leaders and a special guest supplied by Philips corporation.

Recorded LIVE at EMS Expo 2010 in Dallas, Texas last month, this episode was sponsored live by Philips and included a very informative discussion on the different EMS system approaches to handling STEMI, Non-STEMI and other acute coronary syndromes.

EMS Precepting and Preceptor Skills

This week's episode is about the important role that clinical experience, street time, and precepting plays in the EMS educational process. As new EMTs and paramedics are educated and released onto the street either as part of their academy or college program or in new job placements, the time spent shadowing and learning alongside an experienced emergency medical provider is invaluable.

Why, then, does this job have no defined structure or certification associated with it? One of the most important things a student EMT or paramedic will do is pick up the tricks, skills and trade craft of their preceptor while riding their first hours on the road with their patients. The job of preceptor is too valuable to just be handed off to the next guy on the list with at least a year or two under their belts.

Tip This Week --- EMS Review on Antihypertensives

Hypertension is one of the most common chronic illness in the US and also one of the leading causes of heart disease. Before hypertension reaches its complications, patients with hypertension are given medications to be taken everyday to maintain their blood pressure at normal.

There are different antihypertensives given to individual patients. Though these all work to lower the blood pressure, it differs in the mechanism in which they work. The types of antihypertensives include beta blockers, calcium channel blockers, ACE inhibitors, diuretics, adrenergic modifiers, Angiotensin II receptor blockers and other combination medications. It is important for EMS providers to be knowledgeable of these medications to be able to provide correct information for patients with queries or for those who need further understanding of their medication.

Antidepressants, particularly TCAs were one of the most important causes of mortality resulting from poisoning and continue to be responsible for more deaths per prescription than all the other antidepressants put together. They are identified as one of the most common causes of self poisoning and comes in second to analgesics as the most common drug taken in drug overdose resulting to fatality.

There following are the types of antidepressants:

  • Selective Serotonin Reuptake Inhibitors (SSRI)

    • eg. Prozac (fluoxetine), Paxil (paroxetine)

  • Serotonin and Norepinephrine Reuptake Inhibitors (SSNIs)

    • eg. )Effexor (venlafaxine), Cymbalta (duloxetine)

  • Tricyclic antidepressants (TCAs)

    • eg. Elavil (amitriptyline), Tofranil (imipramine)

  • Monoamine Oxidase Inhibitors (MAOIs)

    • eg. Nardil (phenelzine), Parnate (tranylcypromine)

Since antidepressants is one of the leading causes in drug poisoning seen in emergency departments, EMTs and paramedics should be on the lookout for clinical manifestations of antidepressant overdose.

The clinical features of antidepressant overdose include the following:

CNS

  • altered mental status, delirium, psychotic behavior, delirium and hallucinations and later proceeds to lethargy, stupor, and coma.
  • generalized seizures within 1-2 hours of ingestion
  • pyramidal signs
  • rigidity

Cardiovascular

  • Hypotension
  • Dysrhythmias
  • Conduction block
  • Slowed ventricular conduction
  • Sinus Tachycardia
  • Hypertension (early)
  • Cardiogenic shock

Respiratory

  • Acute lung injury
  • Hypoventilation
  • Aspiration pneumonitis
  • ARDS
  • Hypoxia

Anticholinergic effects

  • Hypothermia
  • Agitation (early)
  • Mydriasis
  • Dry skin and/or mucous membranes
  • Hyperthermia
  • Decreased gastric motility/ileus
  • Urinary retention

Emergency plan of management includes:

  • Assess and treat ABC’s as needed
  • Examination of clinical features
  • Use charcoal or gastric lavage if within 1 hour after ingestion
  • Give Sodium Bicarbonate for acidosis, arrhythmias and hypotension
  • IV fluids (inotropes) for hypotension
  • Monitor patient for risk of cardiac arrest and institute seizure precautions

Tip This Week --- Head Injury Assessment

Mild head injuries or concussions is a common reason for ER trips and 911 calls especially for individuals involved in contact sports. As we enter the fall season and football season as well, EMS providers should be ready to respond to increased incidence of these types of injuries. One definition of concussion [from webmd.com] is a condition in which there is a traumatically induced alteration in mental status, with or without an associated loss of consciousness (LOC). A broader definition for concussion is a traumatically induced physiologic disruption in brain function that is manifest by LOC, memory loss, alteration of mental state or personality, or focal neurologic deficits. Concussions usually result in relatively temporary impairment of neurologic function.

In assessing concussions, emergency responders should always be ready with their assessment tools such as the Glascow Coma Scale, to assess the LOC; and the Sport Concussion Assessment Tool (SCAT). Providers should also assess for other injuries and ABCs especially for unconscious patients. This Week --- IRCP Conference in Vail, Colorado

The IRCP started when about 50 delegates from Australia, Canada, Scotland and the United States came together and in Nova Scotia to share ideas on integrating rural EMS providers into rural health care delivery systems. At the meeting, they all discovered that their respective countries had similar rural health issues. It is very interesting to note that all systems which developed “Community Paramedic / Paramedic Practitioner” programs did so as the direct result of public pressure for service or a sudden change in the health care human resources dynamic of their areas. It is also interesting to note that EMS systems were not seen as part of the primary care continuum until most other options were exhausted.

Emergency medicine providers are the frontliners in provision of care being the first to respond to emergency calls. In cases of drug or substance toxicity, one of the first things they do inspect the area for pill bottles or chemical containers that may have been the cause of toxicity. In the absence of such evidence, they will have to rely on their assessment skills and observe the patient's symptoms to determine which substance is most likely involved.

In considering toxicological cause, there are toxic syndromes that we can watch for. Toxic syndromes or toxidromes are groups of signs and symptoms that suggest certain types of drugs or chemicals as the cause. Looking at vital signs and symptoms involving the central nervous system, gastrointestinal system, eyes, skin and mucous membranes, and genitourinary system can provide clues to the toxicological origin of the patient’s illness.

The four toxidromes that health care providers should know about include anticholinergic, cholinergic, opioid and sympathomimetic or stimulant toxidrome.

Tip This Week ---Understanding Allergies

Allergies are reactions of the immune system to certain things that does not bother most other people. Common allergens include pollen, dust mites, mold spores, certain types of food, insect stings and medications.

Allergies can cause a runny nose, sneezing, itching, rashes, swelling or asthma. Symptoms vary. Although allergies can make you feel bad, they usually won’t kill you. However, a severe reaction called anaphylaxis is life-threatening.

There are tests that can be done to determine if a person is hypersensitive to a a substance. Types of tests are in vivo or skin/scratch test, in vitro or the measurement of specific IgE antibodies, and provocation testing where an allergen is introduced one at a time and the subject is observed for presence of reactions.

Though allergic reactions can resolve on its own, the symptoms that it produces may be rather discomforting. Medications that can be given to resolve hypersensitivity include Corticosteroids, Antihistamines, Decongestants, and other medications that work by blocking symptom-causing chemicals released during an allergic reaction such as Montelukast, Cromolyn Sodium and Mast cell stabilizers.

Immunotherapy or allergy shots can also be done to desensitize the patient to decrease or eliminate the need for medications. For patients with severe allergic reactions or anaphylaxis, epinephrine shots may be given.
