Falls in children and adolescents tend to occur in homes, followed by schoolyards and playgrounds. Adults in the other hand tend to get injured from occupational falls. A large percentage of falls in adults have been associated with alcohol use. Older adults aged 65 and above are highly at risk for falls because of factors like poor muscle tone, poor balance, medication use, and frequency of falls increase from environmental factors including poor lighting and lack of safety devices. In assessing fall injuries, EMS providers could determine the extent of injury resulting from falls depending on certain factors like the distance of the fall, the landing surface, orientation on falling, and whether the fall was broken. Emergency responders often see patients who have used street drugs like heroin and cocaine but don't always think about what other drugs or substances that have been mixed in with them. These contaminants are usually added to dilute the drug or to extend the "high". These may or may not change the appearance and taste of the drug and may prove difficult for the user or paramedic to immediately identify the contaminants. For providers attending to patients on street drugs who are presenting unusual signs and symptoms, it would be advisable to coordinate with their local poison centers so they could project what other drugs may have been added. For EMS providers to determine when not to resuscitate, one thing they could do is to check with their state laws. Some states have specific provisions for DNR orders like in form of bracelets, necklaces or cards. Resuscitation can also be withheld in cases like decapitation, decomposition, and trauma where survival is not possible. DNR can also be considered in patients suffering from a terminal illness like cancer in advanced stage. In cases patients do not meet the criteria for DNR (as per state law), EMS providers should initiate all needed interventions for patients in need of resuscitation. Suboxone, is buprenophrine added with naloxone to decrease the likelihood of diversion and abuse of the combination product. Ideal candidates for Buprenorphine therapy are chosen following a criteria and three phases of maintenance therapy are followed. The dose of Buprenorphine given in each phase of the therapy is closely monitored by the physician and adjusted through the progression of each phase until medically supervise withdrawal (formerly called detoxification) is achieved. providers responding to Buprenorphine users with pinpoint pupils, extreme drowsiness, dizziness, blurred vision and slowed breathing should suspect drug overdose. Emergency measures should be directed at providing a patent airway for the patient, careful monitoring of respiratory and cardiac status and immediate transport to a health facility. If overdosed with parenteral form of the drug, paramedics should remember that Naloxone may not be effective in reversing respiratory depression. Doxapram, a respiratory stimulant, may be used. ]]> Sun, 06 Dec 2009 16:44:53 -0500 stemi-imitators-for-ems-and-episode-196 Jamie Davis, the Podmedic STEMI (S-T Elevation Myocardial Infarction) Imitators Review for EMS and Episode 196 of the MedicCast EMS Podcast for EMTs Paramedics and other emergency medical services providers Tip This Week --- False Positives for STEMIs Paramedics and medics (EMT-I) treating cardiac patients in the field are often taught to look for and diagnose S-T elevation myocardial infarctions (STEMI) in the field. EMS providers do this using their heart monitors and performing a 12-lead electro cardiogram (ECG) as well as adding in information from physical assessment and patient history. With more awareness about STEMI recognition and treatment, some cardiac centers are noticing a larger number of "false positive" STEMI patients coming in to their cath labs. The question is now being asked, are we ruling out STEMI imitators carefully enough in the field and in community hospitals. I add the local hospitals because the local ER docs may make the same mistaken diagnosis based on presentation and ECG evidence, too (based on evidence from one article linked below). The two easiest STEMI imitators are the bundle branch blocks. Left bundle branch blocks almost always present with an S-T segment elevation, and while Right bundle branch blocks usually present with S-T segment depression, this may not always be the case. Bundle branch blocks are most easily identified by the presence of a wide QRS segment (greater than .17 seconds). While any patient presenting with symptoms of MI including chest pain, shortness of breathe, diaphoresis, and other signs of shock who has a wide complex QRS segment on their ECG with ST Elevation MIGHT indeed be having an MI - the ECG S-T segment is not diagnostic for this and your system may have to rely on cardiac markers in the blood tests to make a decision. Other typical STEMI imitators include Left Ventricular Hypertrophy, Pericarditis, and Ventricular Aneurysm. Pulmonary Embolism is a sudden blockage in a lung artery. It is usually a result of a blood clot in the leg (DVT) that broke loose and traveled into the lungs via circulation.

About half the people who have pulmonary embolism do not show any symptoms, but clinical manifestations include shortness of breath, chest pain or coughing up blood. Treatment is aimed at breaking up the blood clots and preventing formation of new clots.

The most important thing that can be done in the prehospital setting is to transport the patient to a hospital. As long as no reliable method is available of making a clinical diagnosis of pulmonary embolism (PE) without diagnostic tests, treating PE in a meaningful way in the field will remain difficult. Emergency interventions are aimed at respiratory management and establishment of an IV line.

]]> Sun, 22 Nov 2009 21:34:39 -0500 deep-vein-thrombosis-dvt-and-episode-194 Jamie Davis, the Podmedic Deep Vein Thrombosis (DVT) and Episode 194 of the MedicCast EMS podcast for EMT and paramedic emergency medical providers and students Tip This Week- Deep Vein Thrombosis (DVT) Deep venous thrombosis (DVT) refers to a blood clot embedded in one of the major deep veins of the lower legs, thighs, or pelvis. A clot blocks blood circulation through these veins, which carry blood from the lower body back to the heart. The blockage can cause pain, swelling, or warmth in the affected leg. If this clot is left unresolved, it may dislodge and travel to the lungs via the bloodstream, a more serious problem called pulmonary embolism. Physical symptoms of DVT are similar with the symptoms after the occurrence of trauma. To be able to identify DVT, paramedics should use their critical thinking skills, thorough history taking as well as knowledge on the causes and risk factors. If there is a moderate or high clinical suspicion of DVT an injection of low-molecular-weight (LMW) heparin in doses designed to treat acute DVT can be given. Diagnostic imaging can then be arranged on a more elective basis the following day. In using a stethoscope, it is important that proper placement and technique is done to be able to maximize the use of the device. Selecting an ideal stethoscope that will fit the preferences of t user will also help in the quality of its function. The areas most often auscultated are the lungs and the heart. To be able to identify abnormalities, the provider should be trained and be familiar with the different breath sounds and heart sounds. The field or environment where the assessment is done won't always be ideal and much noise can be present interfering with the sound they are trying to hear. Discussion on New Paramedic Show "Trauma" Last Monday night, members of the EMS community got together online to watch the premier of the new show Trauma. Comments were mostly made on how TV paramedics portray the real paramedics and the accuracy of the procedures that they do. For EMS providers, it is important that these kind of shows get as close to the real thing as possible to educate the public about what emergency medicine is all about and not give them false impressions or expectations. Together with the podmedic were EMS podcasters Greg Friese of EMS Educast and Chris Montera of the EMS Garage with a paramedic consultant from the show as a special guest. One of the actresses, Aimee Garcia (facebook fan page), called in to share her experiences during her preparation for her character portrayal. She said that actual paramedics provided the EMS training for the actors which included the basic EMS procedures. They discuss the effects of the H1N1 pandemic and flu season to EMS providers. Since health providers are on the front line of dealing with illnesses, it is important for them to practice measures to avoid transmitting or further spreading the infection. Every medical professional knows that handwashing is the universal precaution. Proper handwashing every after patient reduces the risk of passing on the virus to the next patient or to other people that the health provider interact with. Other measures like using alcohol-based hand rub can also be helpful. It is unlikely for a health care provider to avoid close contact for people who might be ill or to avoid crowded places especially for those working at the ED. However, they can use protective clothing like masks or gowns, depending on the institution's protocol. Most importantly, the immune system should be given a boost by good health habits including adequate sleep, eating nutritious food, and keeping physically active. ]]> Sun, 30 Aug 2009 19:12:23 -0400 mva-mechanisms-of-injury-and-episode-182 Jamie Davis, the Podmedic MVA Mechanisms of Injury and Episode 182 of the MedicCast Podcast for EMTs, paramedics, EMS students and educators Tip of the Week– MVA Mechanisms of Injury Motor vehicle accidents cause up to a million deaths per year worldwide. According to the WHO it ranks fourth in global health problems. In the US, about 42,000 are killed from MVA and injuries reach a number as high as 3.5 million . These figures are alarming and indicates that incidence of vehicular accidents are high so emergency medicine providers should always be alert in responding to such cases. In relation to the previous episode, Kinematics of Trauma, motor vehicle crashes also follow the laws of physics, meaning, the injuries sustained by the individual involved in the crash would depend on physical factors like the velocity at which the vehicle is moving, involvement of seatbelts and airbags, and the direction at which the injury occurred. Knowing the mechanism of injury in motor vehicle accidents would give the paramedic an edge in performing a thorough assessment just by identifying the type of injury. For example, in frontal crashes with airbags deployed and the patient wearing seatbelts, the paramedic should expect lateral rib fractures with evenly distributed visceral injuries, intestinal injuries from compression of the seatbelt into the body, and pelvic fractures. His book is titled - "Resuscitate: How your community can improve survival from sudden cardiac arrest." For instance, an object traveling at a velocity of 150 m/s would yield a greater impact on collision as compared to the same object moving at 100 m/s, meaning, more damage can be expected from a car that crashes at high speed. Emergency responders should be aware of such concept because kinematics of trauma would give them a head start of what injuries to expect depending on the incident. For example, if a fall victim would fall feet first, injuries to find are fractures associated to the lower extremities including spine compression or what they call as "Don Juan Syndrome". The cornerstone of assessment is early consideration of kinematics to predict hidden injuries. One thing that paramedics should do in such instances before attempting to transport the patient is to immobilize the spine. In performing spinal immobilization, the paramedic should pay special attention to the c-spine. If it is not in neutral position, an attempt should be made to achieve proper alignment. If the patient is able to move, they should move their neck into the line. If there is any pain, neurological deterioration or resistance to movement the procedure should be abandoned and the neck splinted in the current position. There is a criteria set in determining candidates for spinal immobilization. Ultrasonography is one of the most commonly used tools for assessing patients in a facility setting and has been shown to reduce morbidity and mortality in critically ill patients. The use of ultrasound technology in the pre-hospital setting has the potential to improve trauma, cardiac and obstetric care. Being able to perform the diagnostic procedure even before reaching the hospital saves precious time for the patient. As of now, it is only accessible in the hospitals in most areas but recent technological advances have made hand held ultrasound devices available in helicopter emergency medical services (HEMS) and ground ambulances. Several research projects on HEMS ultrasound and ground EMS ultrasound are ongoing in the United States and countries across Europe. Tip of the Week– CPAP for EMS Providers

Continuous Positive Airway Pressure (CPAP)is a non-invasive procedure usually given in the prehospital setting aimed at improving the status of a patient with respiratory problems by reducing work of breathing. In some patients, it is used as an alternative for intubation.

CPAP is usually given in conjunction with emergency medications such as nitrates, diuretics or bronchodilators depending on the respiratory problem the patient is having.

Indications for CPAP are:

  • CHF/Acute Pulmonary Edema
  • Acute Respiratory Failure
  • asthma/reactive airway disease,
  • near drowning,
  • COPD,
  • acute pulmonary edema (cardiogenic and non cardiogenic)
  • pneumonia

Administration should start at lowest setting (0-2 cmH20) and titrated upward to 10 cmH2O.

When providing CPAP, the paramedic should verbally calm the patient and provide coaching since it may induce anxiety.

Complications that emergency medicine providers should anticipate include drop in blood pressure, pneumothorax, gastric distention and vomiting, and corneal drying.

Contraindications to CPAP are:

  • Unconscious, Unresponsive, or inability to protect airway.
  • Inability to sit up
  • Respiratory arrest or agonal respirations (Consider Intubation)
  • Persistent nausea/vomiting
  • Hypotension- Systolic Blood Pressure less than 90 mmHg
  • Inability to obtain a good mask seal
  • Pathologic
  • Suspected Pneumothorax
  • Shock associated with cardiac insufficiency
  • Penetrating chest trauma
  • Facial anomalies /trauma/burns
  • Closed Head Injury
  • Has active upper GI bleeding or history of recent gastric surgery
  • Vomiting

CPAP therapy should be given continuously to obtain maximum benefits. In the event that it should be discontinued, Intermittent Positive Pressure Ventilation or Endotracheal intubation should be considered.

From simple errands at home using common appliances to large scale construction projects with state-of-the-art power tools. Individuals constantly making use of electricity, especially workers, are prone to electrical injuries because of the environment that they are in and according to the National Institute for Occupational Health and Safety (NIOSH), an average of one worker is electrocuted on the job on every day of every year. There are four main types of electrical injuries that EMTs and paramedics should be watching for in their patients. The most common shock-related non-fatal injury is a burn and there are three types. Electrical burns (from touching electrical wires), arc burns (from currents arcing through the air ), and thermal contact burns. In relation to electricity, lightning strikes can also be a source of electrical injury. According to the US Centers for Disease Control and Prevention (CDC), approximately 100 of every 500 fatalities caused by electricity are the result of lightning strikes. Although related, there are significant differences in injury patterns and pathophysiology. In a lightning strike there is the possibility of cardiac arrest, keraunoparalysis (transient paralysis associated with extreme vasoconstriction and sensory disturbances of one or more extremities), blunt trauma , and superficial burns. Treatment of the electrical burn patient starts with basic care including ensuring airway patency, respiratory adequacy, managing cardiac arrest if present. Because of the hidden injuries caused when the current passed through the patient's internal tissues, no assumptions should be made based on location of entry or exit wounds. O2, IV, and ECG monitoring are where EMS treatment should begin once initial assessment and life threats have been dealt with. Treat visible burn injuries per EMS protocol and contact online medical control and your local burn center for specific instructions for your patient. ]]> Sun, 05 Jul 2009 22:43:11 -0400 antidepressant-overdose-for-paramedics-and-episode Jamie Davis, the Podmedic Episode on Antidepressant Toxicity for EMTs and Paramedics Antidepressants, particularly TCAs were one of the most important causes of mortality resulting from poisoning and continue to be responsible for more deaths per prescription than all the other antidepressants put together. They are identified as one of the most common causes of self poisoning and comes in second to analgesics as the most common drug taken in drug overdose resulting to fatality. There following are the types of antidepressants: * Selective Serotonin Reuptake Inhibitors (SSRI) o eg. Prozac (fluoxetine), Paxil (paroxetine) * Serotonin and Norepinephrine Reuptake Inhibitors (SSNIs) o eg. )Effexor (venlafaxine), Cymbalta (duloxetine) * Tricyclic antidepressants (TCAs) o eg. Elavil (amitriptyline), Tofranil (imipramine) * Monoamine Oxidase Inhibitors (MAOIs) o eg. Nardil (phenelzine), Parnate (tranylcypromine) Since antidepressants is one of the leading causes in drug poisoning seen in emergency departments, EMTs and paramedics should be on the lookout for clinical manifestations of antidepressant overdose. The clinical features of antidepressant overdose include the following: CNS * altered mental status, delirium, psychotic behavior, delirium and hallucinations and later proceeds to lethargy, stupor, and coma. * generalized seizures within 1-2 hours of ingestion * pyramidal signs * rigidity Cardiovascular * Hypotension * Dysrhythmias * Conduction block * Slowed ventricular conduction * Sinus Tachycardia * Hypertension (early) * Cardiogenic shock Respiratory * Acute lung injury * Hypoventilation * Aspiration pneumonitis * ARDS * Hypoxia Anticholinergic effects * Hypothermia * Agitation (early) * Mydriasis * Dry skin and/or mucous membranes * Hyperthermia * Decreased gastric motility/ileus * Urinary retention Emergency plan of management includes: * Assess and treat ABC's as needed * Examination of clinical features * Use charcoal or gastric lavage if within 1 hour after ingestion * Give Sodium Bicarbonate for acidosis, arrhythmias and hypotension * IV fluids (inotropes) for hypotension * Monitor patient for risk of cardiac arrest and institute seizure precautions no 41:30 Anatomy of Abdominal Pain for EMTs & Paramedics and Episode 173 http://www.mediccast.com/blog/2009/06/28/anatomy-of-abdominal-pain-for-emts-paramedics-and-episode-173/ ProMedNetwork.comWelcome to Episode 173

Abdominal pain, also known as stomach ache, tummy ache, or belly pain, is one of the most common complaints of people seeking medical help in the ER. In the U.S. Government Census Data on ER visits in the year 2005, abdominal pain figures are high on the list for adults.

Paramedics and EMTs see many different types of patients whose chief complaint is abdominal pain every day. But one person's abdominal pain is not the same as another person's pain. It is important to know which organs are present in the various quadrants and what each different area's pain means.

Tip of the Week– Respiratory Management for EMTs & Paramedics

Good patient management goes way beyond just making an assessment and giving a treatment. In order to graduate from being an okay EMT or paramedic to becoming a great paramedic you need to go above and beyond.

How far above and beyond?

Actually, not all that far. This week, I'll discuss respiratory patients but you can apply these techniques to all of your patients. When we discuss good patient care, many of us assume that means the mechanics of taking care of a patient. Taking vital signs, doing a head to toe assessment, getting a good patient history, and so on.

All of us need to be good at patient care basics but we also need to remember that our patients are more than just a collection of signs and symptoms and a disease that we treat. They also have hopes and fears, family members and friends, and they are people who hope that you will help them live to see more of all of those things.

When you care for a patient, remember to care for the whole patient. Develop a list of good questions to ask when you have a patient to help take their mind off of their problems. You might have to carry some of the conversation for respiratory patients so be prepared to tell an appropriate funny story. Lightening the mood can do a lot to alleviate respiratory anxiety and decrease workload on the body's systems.

Got a grandparent in the back of the unit, they all love to talk about their children and grandchildren. Ask them to tell you about their grand kids. That distraction can help reduce the level of distress. Use your skills as an EMT or paramedic, too, but remember that your patient has feelings. Don't be that provider who gives a neb, puts the patient on oxygen and then ignores them all the way to the hospital.

Tip of the Week– Ciguatera Fish Poisoning

About Ciguatera

Ciguatera fish poisoning (or ciguatera) is an illness caused by eating fish that contain toxins produced by a marine microalgae called Gambierdiscus toxicus. People who have ciguatera may experience nausea, vomiting, and neurologic symptoms such as tingling fingers or toes.

They also may find that cold things feel hot and hot things feel cold. Ciguatera has no cure. Symptoms usually go away in days or weeks but can last for years. People who have ciguatera can be treated for their symptoms.

Fish like barracuda, black grouper, blackfin snapper, cubera snapper, dog snapper, greater amberjack, hogfish, horse-eye jack, king mackerel, and yellowfin grouper have been known to carry ciguatoxins.

Three interview segments this week to feature the three products that I got most excited about during this year's EMS Today expo hall run. Each of these products represents a different part of the EMS industry and features different ways of innovating in the emergency medical services field.

Amy Turley, inventor of the Turley Backboard pad is a paramedic herself and designed this product to be durable, field tough and long lasting while providing superior padding and pressure point protection to patients who are placed on a long backboard. Each pad rolls up to fit in a small space and they can be heated or cooled to assist with patient care in that way as well. Read the full review at the link above.

The MyClyns personal protective spray and the ViralClyns mask system are both great ways to protect us in the field and give us some peace of mind, too. EMTs and paramedics can use the masks to protect themselves before an exposure and the MyClyns spray will provide some immediate treatment if you are exposed to a splash. The spray is antimicrobial and may be safely sprayed in the eyes, mouth, or in an open wound immediately post exposure to lessen chances of infection. While not taking the place of post exposure measures in your agency or hospital, it certainly provides some immediate peace of mind until you can get cleaned up.

Finally, I'm really excited about the Lifepak 15's updates and improvements. Listen while I chat with Katie Liberto of Physio Control about the new monitor/defibrillator and she tells me about all the ways they've made even tougher and more durable than the Lifepak 12 and still pack in new features and monitoring capabilities. Great job from my personal favorites in the monitoring field.

Every year 30,000 to 40,000 people in the U.S. die from our regular seasonal flu but the media does very little reporting on the tragedy of those deaths. The national agencies like the CDC and Health and Human Services have all responded with an appropriate, measured response to a developing situation of concern. So what are you going to learn from all of this as an EMT and paramedic on the street? Here is a list of my top 4 lessons learned: 1. Where to go to get high quality reliable information. (Check out my article here at EMSLive.com on top 5 things for EMTs to know about swine flu.) 2. Learn the details of your own infection response plan in your region. (You should know by now or you will be asking soon, right?) 3. Review the little used parts of our personal protective gear (PPE). Tip of the Week -- An NREMT Review of Tourniquet Application

The NREMT updated both the ALS and BLS skills stations this year with the major change being the alteration of the "Bleeding Control and Shock Management Station." Previously, EMTs and paramedic students were required to first apply direct pressure, elevate the injured area, utilize arterial pressure points and then after all of those interventions failed, apply a tourniquet.

New evidence collected in recent years from the battlefields of Iraq and Afghanistan have proven the effectiveness and safety of applying tourniquets earlier in the process. Now, the NREMT is testing EMTs and Paramedics to first apply direct pressure and if the bleeding doesn't stop, to move directly to application of the tourniquet.

Previously, the fear of using a tourniquet centered around the thought that using a tourniquet on an extremity meant that the limb was lost and an amputation was certain simply because of the tourniquet application. That is now understood not to be the case.

The muscle and bone tissues of the extremities may be revived as long as 5 to 6 hours later. Since few if any EMS agencies in the U.S. have a normal transport time greater than 5 hours, the tourniquet becomes a more appropriate action to take.

Review your tourniquet rules using your own protocols along with the reference articles I have included below for all EMT and Paramedic personnel whether they are students or veteran providers.

This Month's MedicCast Live Show EMS Radio Consults

A special thanks to Dr. Ginger Campbell of the Brain Science Podcast among other shows (find Ginger's stuff at GingerCampbellMD.com). She suggested the topic for this week's episode and was gracious enough to come on the show live and share her thoughts about receiving radio patient information in the ER.

Also, a thank you to Dr. Craig Corey for taking the time to write his thoughts down even though he couldn't be on the show live. Craig is the inventor of the OxyPhone pediatric nebulizer device. You'll find that and Dr. Corey's home page at OxyPhone.com.

The central point brought out by both of these ER docs was that we must get to the point of the call quickly, focusing on the key problem, immediate life threats and interventions taken, time to the hospital, etc. and then let the ER ask any follow up questions. Vitals are important but not as important as patient's chief complaint and your impression of the stability of the patient.

Ginger also stressed the importance of talking to the physician after the call to learn from what you saw or did during the transport to the hospital. She said that her respect for the EMT or paramedic rises significantly when they want to learn from the calls - good and bad.

Listener Jon sent in an email about a 2 year old patient with SMA and how difficult it was to prepare a treatment plan for this patient when he and his partner had no clue about what the disease was. I have included a link to the National Institutes of Health (NIH) website page on SMA that includes a ton of great medical and patient resources on this disorder.

I would also remind all of you that you can always get a lot of information on rare disorders from parents and caregivers of special needs patients. These people know a lot about what is going on. You possess emergency care knowledge. Put the two together and you can very effectively treat your patients. Be honest when you don't know about a particular disorder or disease and, who knows, you might learn something.

Household Chemicals and Carbon Monoxide

  • Always read the label before using a product that may be poisonous.
  • Keep chemical products in their original bottles or containers. Do not use food containers such as cups, bottles, or jars to store chemical products such as cleaning solutions or beauty products.
  • Never mix household products together. For example, mixing bleach and ammonia can result in toxic gases.
  • Wear protective clothing (gloves, long sleeves, long pants, socks, shoes) if you spray pesticides or other chemicals.
  • Turn on the fan and open windows when using chemical products such as household cleaners.

]]> Sun, 29 Mar 2009 13:31:06 -0400 closed-head-injuries-and-episode-161 Jamie Davis, the Podmedic Head Injury Review for paramedics and EMTs focusing on epidural and subdural hematomas Tip/Trick of the Week- Closed Head Injuries The two common types of brain bleeds following a traumatic injury that includes a blow to the head are epidural and subdural hematomas. For paramedics and emergency medical technicians encountering these injuries in the field, diagnosis can be difficult. Often, the patients with these head injuries present with no significant side effects initially and will want to refuse treatment. Because of the nature of these hemorrhages, the patients can deteriorate over time and become incapacitated and die from a lack of treatment. EMS providers in the field must use careful history taking and assessment to raise their index of suspicion and work with online medical direction to convince these individuals that they need to be checked out in the ER or trauma center. The only definitive diagnostic tools are the use of medical imaging of the skull to look for signs of internal bleeding. Until this is done, there is no way of determining if there is a bleed present or not. Some patient populations have a higher risk for bleeding anyway and need even more careful assessment. Tip/Trick of the Week- Orthostatic Vital Signs (assessing for hypovolemia)

Taking orthostatic vital signs on the scene, before, and after fluid therapy can be a big help to the ER when you are bringing in a patient who is complaining of dizziness, light headedness, or syncopal episodes and there is no clear cardiac cause such as bradycardia or other arrhythmia.

Orthostatic vital signs, also known as tilt testing or postural vitals, is an assessment tool used to identify patients with a fluid volume deficit such as dehydration. It assesses the patient in multiple positions and tests the cardiovascular response to the changes in position after a short period of time (usually 1 to 2 minutes).

Changes of greater than 20 mm Hg systolic in the blood pressure reading or greater than 10 to 20 BPM in the heart rate after a postural change may indicate that the person is low on fluids and may be a candidate for a fluid bolus. (Refer to your own protocols or to online medical control before administering such a bolus)

]]> Sun, 22 Mar 2009 18:33:11 -0400 orthostatic-vital-signs-and-episode-160 Jamie Davis, the Podmedic Orthostatic Vital Signs review for EMTs and paramedics Tip/Trick of the Week- Orthostatic Vital Signs (assessing for hypovolemia) Taking orthostatic vital signs on the scene, before, and after fluid therapy can be a big help to the ER when you are bringing in a patient who is complaining of dizziness, light headedness, or syncopal episodes and there is no clear cardiac cause such as bradycardia or other arrhythmia. Orthostatic vital signs, also known as tilt testing or postural vitals, is an assessment tool used to identify patients with a fluid volume deficit such as dehydration. It assesses the patient in multiple positions and tests the cardiovascular response to the changes in position after a short period of time (usually 1 to 2 minutes). Changes of greater than 20 mm Hg systolic in the blood pressure reading or greater than 10 to 20 BPM in the heart rate after a postural change may indicate that the person is low on fluids and may be a candidate for a fluid bolus. This Month's MedicCast Live Show on Collegiate EMS

This month, the MedicCast Live looked at the unique group of community EMS volunteers that make up the collegiate EMS provider. These EMTs and paramedics are college students who provide volunteer emergency medical services as first response units and transport units for their classmates at colleges and universities across the country.

We were very pleased to have two representatives from the NCEMSF (National Collegiate EMS Foundation). NCEMSF President Dr. George J. Koenig, D.O. and Vice President Scott Savett, PhD both active responders in collegiate EMS during their undergraduate careers. Dr. Koenig is a trauma fellow at Johns Hopkins University Hospital and Dr. Savett is chemist and informatics implementation specialist in the pharmaceutical industry.

We also had long time listeners David Cawston formerly of the University of Vermont response team and Woody Parker of St. Lawrence University EMS in upstate New York. We also had the pleasure of hosting several other collegiate EMS providers in the chatroom and on the phone lines during the show.

We discussed confidentiality issues and HIPAA rules, common funding issues for these lightly funded organizations, interactions and training with surrounding services, and other current focuses of the NCEMSF and their member organizations.

I urge all of you to listen to this episode with an ear towards how important it is to work with neighboring organizations in training, joint operations and emergency planning. These issues came up during the discussions on several occassions and don't just pertain to collegiate EMS. These problems are faced and affect all of us.

Tip of the Week: Hallucinogenic Plants

As students and young adults try to find ever more creative ways to get high, EMS providers like EMTs and paramedics will have to refresh their knowledge about the different behaviors in their region. This week, Lisa Booze of the Maryland Poison Center shares her knowledge of some of the common plants used by teens to create hallucinogenic effects.

Among them is one named Jimsom Weed. Follow the link above to a full article on Jimsom Weed intoxication using the link above. Whatever the plant based culprit, it will fall to the EMS provider on the scene to use some detective skills during their patient assessment to determine what the specific patient presentation might be pointing to. Relay this information to the local poison center and your online medical control service to work out a treatment plan.

Some items to look for in and around the patient's location:

  • seeds
  • stems
  • flower petals
  • leaves
  • dried plant material
  • baggies of plant or organic materials

Treat any patient with an unknown cause of altered mental status with the basics first. Follow your ABCs to treat any obvious life threats first and then try to get what info you can from bystanders or family members (if not from the patient themselves). If the patient is conscious, attempt getting a history from them but try to do so once alone in the back of the ambulance without family or police units around.

In this section as we look at part two of the two part series on EMS Humor, I wanted to include some of the great abreviations and acronyms that people in the live chat room were throwing out there for everyone to laugh at. While these are all very amusing, I think we should all be aware that they are not to be used in acutal patient documentation.
