Tip This Week --- Behavioral and Psych Patient Assessment
Assessing behavioral and psychiatric patients may prove to be a challenge for EMS providers. In asking for information, the interviewer should not only look into what the patient is saying but also on the non-verbal cues and the cohesion of ideas and derive their assessment from that. The provider should also be aware that not all alterations in behavior are from psychiatric causes. They should also consider and assess for low blood sugar, lack of oxygen, head trauma, extreme temperatures and substance use.For both behavioral and psychiatric patients, emergency responder and patient safety should always be first on the list. Though not all behavioral patients may be a threat, it will never hurt to always be careful.

This Week --- IRCP Conference in Vail, Colorado
The IRCP started when about 50 delegates from Australia, Canada, Scotland and the United States came together and in Nova Scotia to share ideas on integrating rural EMS providers into rural health care  delivery systems. At the meeting, they all discovered that their respective countries had similar rural health issues. It is very interesting  to note that all systems which developed “Community Paramedic / Paramedic Practitioner” programs did so as the  direct result of public pressure for service or a  sudden change in the health care human resources  dynamic of their areas. It is also interesting to  note that EMS systems were not seen as part of the primary  care continuum until most other options were exhausted.

Now, the IRCP is on its 6th Convention which was held at Vail, Colorado. Tip This Week --- Lisa Booze Tox Tidbit on Toxidromes
Emergency medicine providers are the frontliners in provision of care being the first to respond to emergency calls. In cases of drug or substance toxicity, one of the first things they do inspect the area for pill bottles or chemical containers that may have been the cause of toxicity. In the absence of such evidence, they will have to rely on their assessment  skills and observe the patient&#039;s symptoms to determine which substance is most likely involved.

In considering toxicological cause, there are toxic syndromes that we  can watch for. Toxic syndromes or toxidromes are  groups of signs and symptoms that suggest certain types of drugs or  chemicals as the cause. Looking at vital signs and symptoms involving  the central nervous system, gastrointestinal system, eyes, skin and  mucous membranes, and genitourinary system can provide clues to the  toxicological origin of the patient’s illness.

The four toxidromes that health care providers should know about  include anticholinergic, cholinergic, opioid and sympathomimetic or  stimulant toxidrome.

Tip This Week ---Understanding Allergies
Allergies are reactions of the immune system to certain things that  does not bother most other people. Common allergens include pollen, dust  mites, mold spores, certain types of food, insect stings and  medications.

Allergies can cause a runny nose, sneezing, itching, rashes,   swelling or asthma.  Symptoms vary. Although allergies can make you feel  bad, they usually  won’t kill you. However, a severe reaction called  anaphylaxis is  life-threatening.

There are tests that can be done to determine if a person is  hypersensitive to a a substance. Types of tests are in vivo or  skin/scratch test, in vitro or the measurement of specific IgE  antibodies, and provocation testing where an allergen is introduced one  at a time and the subject is observed for presence of reactions.

Though allergic reactions can resolve on its own, the symptoms that  it produces may be rather discomforting. Medications that can be given  to resolve hypersensitivity include Corticosteroids, Antihistamines,  Decongestants, and other medications that work by blocking  symptom-causing chemicals released during an allergic  reaction such as  Montelukast, Cromolyn Sodium and Mast cell stabilizers.

Immunotherapy or allergy shots can also be done to desensitize the  patient to decrease or eliminate the need for medications.</itunes:summary>

Tip This Week --- Reactive Airway Disease
Asthma is a disorder caused by inflammation in the airways (called   bronchi) that lead to the lungs. This inflammation causes airways to   tighten and narrow, which blocks air from flowing freely into the lungs,   making it hard to breathe. Symptoms include wheezing, breathlessness,   chest tightness, and cough,  particularly at night or after  exercise/activity.

Asthma usually has many causes or triggers. The triggers that may  result to the occurrence of asthma include respiratory infections,  allergens, irritants, weather changes, activities, GERD, upper airway  inflammation and emotional factors.

A high percentage of children with asthma also has some type of  allergy, the allergy isn’t always the primary cause of asthma. Even if  allergies  are not primary triggers for asthma they can  still make  symptoms worse.

The goals of asthma therapy are to prevent  chronic and troublesome  symptoms, to maintain lung function  as close to normal as possible,  maintenance of normal  physical activity levels, to prevent recurrent   asthma attacks and to reduce the need for emergency department visits or   hospitalizations, and to provide medicines that give the  best results  with the fewest side effects.

Medications for asthma include inhaled corticosteroids, and  long-acting bronchodilators.

Featured this week- EMS Radio Consults

A special thanks to Dr. Ginger Campbell of the Brain Science Podcast  among other shows (find Ginger’s stuff at GingerCampbellMD.com (http://gingercampbellmd.com/)). She suggested the topic for  this week’s episode and was gracious enough to come on the show live and  share her thoughts about receiving radio patient information in the ER.

Also, a thank you to Dr. Craig Corey for taking the time to write his  thoughts down even though he couldn’t be on the show live. Craig is the  inventor of the OxyPhone pediatric nebulizer device. You’ll find that  and Dr. Corey’s home page at OxyPhone.com (http://oxyphone.com/).

The central point brought out by both of these ER docs was that we  must get to the point of the call quickly, focusing on the key problem,  immediate life threats and interventions taken, time to the hospital,  etc. and then let the ER ask any follow up questions. Vitals are  important but not as important as patient’s chief complaint and your  impression of the stability of the patient.

Ginger also stressed the importance of talking to the physician after  the call to learn from what you saw or did during the transport to the  hospital. She said that her respect for the EMT or paramedic rises  significantly when they want to learn from the calls – good and bad.

Children are naturally curious with the objects they see around them. Most often, whatever they pick up goes straight to their mouth which makes them prone to things like choking, GI disorders and yes, poisoning.

To be able to initiate interventions, it is important to know the agent that the child ingested. EMS providers should look around and be aware of the objects in the child&#039;s environment and the substances found that they could possibly ingest. It is also important to assess carefully for the different signs and symptoms that the child is exhibiting. If the child is symptomatic, obtaining the history of ingestion is also helpful.

Since lead and iron are some of the most common substances that cause pediatric poisonings, providers should also be aware of the signs and symptoms and emergency treatment of lead and iron poisoning. Teaching the parents or caregivers about how and where they can get poisoned by these substances, how they should store chemicals, as well as how they educate the children about medications would be key to the prevention of pediatric poisonings.
