This is a story of a New York paramedic EMT who saved a boy’s life 19 years ago and has been really an important aspect of that family’s life ever since. He has stayed up with what’s going on with that boy’s life, the boy just graduated from college and the paramedic was there to attend his college graduation party 19 years later and that’s exciting. The paramedic inspired the mom to become a nurse and go and get the training necessary to become a nurse.

I picked up this article out of the Massachusetts area about two state troopers who delivered a baby alongside the highway. It just goes to point out this article’s pretty interesting. A woman was on her third baby, she knew she was in active labor. According to the article here, the troopers contacted emergency services when they found the woman on the side of the road and called for assistance and they were the first two on the scene and the baby was coming and so they had to step and do what needed to be done.

Tip This Week --- Lisa Booze Tox Tidbit on Toxidromes
Emergency medicine providers are the frontliners in provision of care being the first to respond to emergency calls. In cases of drug or substance toxicity, one of the first things they do inspect the area for pill bottles or chemical containers that may have been the cause of toxicity. In the absence of such evidence, they will have to rely on their assessment skills and observe the patient's symptoms to determine which substance is most likely involved.

In considering toxicological cause, there are toxic syndromes that we can watch for. Toxic syndromes or toxidromes are groups of signs and symptoms that suggest certain types of drugs or chemicals as the cause. Looking at vital signs and symptoms involving the central nervous system, gastrointestinal system, eyes, skin and mucous membranes, and genitourinary system can provide clues to the toxicological origin of the patient’s illness.

The four toxidromes that health care providers should know about include anticholinergic, cholinergic, opioid and sympathomimetic or stimulant toxidrome.

This is an article I picked up out of British Columbia. Another tragic story, not one of our own going down but a bull rider was succumbed to injury sustained while bull riding. Died at the hospital after he got there but there’s a complaint here that the ambulance cuts in the local service caused his death, that he might not have died had there been an ambulance on scene at the rodeo. Apparently, the two ambulances for this community were on a response to a motor vehicle accident at another part of the district and were nowhere near the rodeo when this guy was thrown off the bull and then trampled by the bull and he got up and struggled to the side where he collapsed, so probably multisystem trauma, many injuries. We don’t know what exactly killed this individual but I can safely say he wasn’t killed by an ambulance, it wasn’t an ambulance that ran over him, he wasn’t trying to ride on an ambulance, he was riding on an angry bull.

That’s right, in the state of Maryland our public service flight medics are state troopers and they fly around and not only are they involved in their duties as a police officer involved in maybe car chases, tracking down fugitives, search and rescue, they also are our primary trauma and medical emergency transport by air in the state of Maryland.

So this is down in Texas and unfortunately, a paramedic candidate who was 48 years old died during a training session apparently from sudden cardiac arrest. Now he was doing a skills station where he was having to- I guess early in the hiring process, he had to demonstrate he could do some basic things. One of the tests was to carry or move the motorized cot across a grassy area with a 185 pound mannequin on it.

It’s summertime here in my area in North America and the Northern hemisphere in general so those of you down in Australia, New Zealand, South Africa and other places that listen to the show, bear with me, I know it’s winter time where you are but we do need to address some things here. Think about what’s going on in your community and kind of apply the same kind of concepts but here where we are, we have kids involved in sports, we have a lot more kids out of school so they are out on the street and doing different activities, skateboarding, roller blading, riding their bikes. Are they using the proper safety equipment for the sports activity or the recreational activity? If they aren’t, they need to be. In this article over at the WebMD site that I found is talking about sports safety: helmets save heads. I love that article title; I think it’s a great headline.

The second hottest June on record was recorded this past month and this has caused a whole bunch of heat related injuries. Temperatures averaging in North Carolina, according to this article, 5 degrees above average for the month, that’s about what it was in my region. There’s no other way to say it, it was hot!

This story is about a new 911 dispatch program that is hoping to ease overcrowding. Now we’ve talked about this in the news on several different occasions but every time a new system starts utilizing something like this, I like to bring it up because I think this is really the standard by which we should all be using and moving certainly in very busy systems in suburban and metropolitan areas. This is a 911 program that will be instituted by Louisville Metro Emergency Medical Services and this a program under which the low priority calls- somebody calling in with ear ache or a nosebleed or whatever the case may be, they call in to 911, they get the low priority call but usually we dispatch an ambulance to them. How can we change that?

This story is looking at this situation in Massachusetts. You know, I think a few bad apples can make all of us look bad pretty quickly but what about what’s going on up there? Maybe you haven’t become aware, State of Massachusetts public health officials are looking over the EMT re-certification standards and procedures for how those certifications are verified over the last few years because 200 emergency medical responders in different communities, this isn’t just isolated to one system.
