Tip This Week — Interview with Greg Adams of Medic Alert Foundation
One common controversial issue in EMS is whether to attempt resuscitation on a patient or not. For EMS providers to determine when not to resuscitate, one thing they could do is to check with their state laws. Some states have specific provisions for DNR orders like in form of bracelets, necklaces or cards. Resuscitation can also be withheld in cases like decapitation, decomposition, and trauma where survival is not possible. DNR can also be considered in patients suffering from a terminal illness like cancer in advanced stage.
In cases patients do not meet the criteria for DNR (as per state law), EMS providers should initiate all needed interventions for patients in need of resuscitation. A DNR decision is irrevocable and should be made only when all legal and ethical factors have been taken into account.
